Continuing To Be Me offers flexible person-centred support, occupational therapy, assessment, intervention, and training for people living with dementia, their families, and carers.

We also provide dementia consultancy, training and education to a range of providers and settings including NHS trusts and care homes.

What is Occupational Therapy?

Occupational Therapy enables people to participate in daily life to improve their health and wellbeing.

Daily life is made up of many occupations, which includes self care (getting ready to go out or eating a meal), being productive (going to work or volunteering) and leisure (socialising with friends or doing hobbies).

How can Occupational Therapists help those affected by dementia?

As Occupational Therapists we can can work with you to overcome the barriers that prevent you from doing what matters to you and support you to live well with dementia. This is the ethos behind Continuing to be Me.

We understand that each person’s journey with dementia is different and each family’s experience is unique.

Occupational therapy can help you to:

  • Use your strengths and abilities to stay active.

  • Adopt strategies and techniques to continue with daily occupations (activities).

  • Make changes to make life easier and to live safely in your home.

  • Access your community, getting to the shops and local facilities.

  • Continue with valued roles, such as working or caring for others.

  • Advise family and friends on how to support you to live well with dementia.

Dementia Services for Individuals & Families

We work directly with clients with dementia and families or carers affected by dementia to provide individual assessment and intervention. Our aim is to identify our clients needs and work with them to set achievable goals. Our assessments and interventions usually take place in a person’s own environment.

Occupational Therapy for Dementia

Provide help and training in undertaking daily living activities such as bathing, dressing, eating, and participating in a favourite hobby or accessing community activities to improve daily health and wellbeing. We may support with structuring time, and creating and maintaining routines.

Understanding Care Needs

We can assess and advise how best to meet a person’s care needs, manage risk issues and the best environment throughout their dementia. We offer support and assessment when transitions in care environments are being considered and during this process.

Environment

We can support you to improve and adapt your environment to help compensate for impaired memory, learning and reasoning skills. We can recommend dementia-friendly design ideas and can make longer term recommendations in terms of ensuring your home will meet your needs both now and in the future.

Creative Interventions

We may recommend and provide creative arts interventions, offer life story work and life history profiling and reminiscence work. We can also support you to access creative activities within the community.

Assistive Technologies

Advise and recommend electronic assistive technology, equipment and adaptations to support independence and decrease risk in the environment, including your home.

Cognitive Interventions

Provide goal oriented cognitive rehabilitation including home based memory rehabilitation, which has been shown to improve functioning for people with early stage dementia. We also provide cognitive stimulation therapy.

Support & Advice

We can provide family members and carers support, advice and education on how to support someone living with dementia and the importance of looking after yourself as a carer.

Mobility

Assess mobility, including falls and provide advice and intervention to support independence and help reduce risk.

Dementia Services for Groups & Organisations

We support a range of organisations by providing consultation, education and training for dementia.

  • CTBM is an ideal partner for bespoke projects where dementia matters.

    We can support individuals and organisations from a range of sectors.

  • We provide Tier 1 and Tier 2 dementia training.

  • Training sessions available on:
    • Dementia
    • Depression
    • Anxiety
    • Delirium

  • We will undertake an environmental audit to look at the suitability for people who have dementia.

    We will produce a report and an action plan with recommendations of how the environment could be maintained and improved.

  • We can provide training on how to engage and promote activity in your care home setting including;

    • This is me – Personal profiling, using the tool kit, interactive training, choosing activities, level of concentration, level of cognition, sensory challenges

    • Activity information and ideas for residents, families and carers, interest checklist, 5 minutes, 10 minutes, longer activity ideas

  • CST is an evidence based brief treatment for people with mild to moderate dementia.

    We offer individual and group CST sessions - which involves 14 or more sessions of themed activities.

  • Training on the theory and use of the Pool Activity Level within your setting. The Pool Activity Level (PAL) instrument is widely used as the framework for providing activity-based care for people with cognitive impairments, including dementia.

    The instrument is recommended for daily living skills training and activity planning in the National Institute for Clinical Excellence Clinical Guidelines for Dementia (NICE 2006), and has been proven valid and reliable by a recent research study.
