Personalised dementia care to support wellbeing and independence

Occupational Therapy

Maximising independence, safety, and wellbeing in everyday life. I support people with dementia in daily activities, using a person-centred, strengths-based approach.

Provide help and training in undertaking daily living activities such as bathing, dressing, eating, and participating in a favourite hobby or accessing community activities to improve daily health and wellbeing. We may support with structuring time, and creating and maintaining routines.

“Your suggestions helped Mum keep doing things for herself for much longer. It meant she still felt like herself.”

Cognitive Support

Maintaining independence through personalised memory strategies. I help individuals manage daily tasks with confidence using prompts, routines, and memory aids tailored to their stage of dementia.

We provide goal oriented cognitive rehabilitation including home based memory rehabilitation, which has been shown to improve functioning for people with early stage dementia. We also provide cognitive stimulation therapy.

“The memory book and calendar routine have helped Mum get through the day without as much worry. She’s now dressing herself again.”

Home Environment Support

Creating safer, simpler spaces that reduce confusion and support independence. I assess the home and recommend practical, dementia-friendly changes like signage, lighting, and layout adjustments and aids and equipment. We can support you to improve and adapt your environment to help compensate for impaired memory, learning and reasoning skills. We can recommend dementia-friendly design ideas and can make longer term recommendations in terms of ensuring your home will meet your needs both now and in the future.

“You showed us how a few simple changes – like clearer labels and better lighting – could make a huge difference. Dad is now moving around much more safely.”

Meaningful Activity At Home

Focused activity sessions that support daily engagement, identity, and wellbeing in the comfort of home. Tailored activities bring fun, purpose, and calm at any stage of dementia.

“Mum really came alive during your sessions. Singing, painting, and just talking about old times gave her real joy again and gave us some breathing space too.”

Family & Carer Support

Practical advice and emotional support for carers. I offer personalised sessions to help families feel more confident in managing behaviours, routines, and the emotional load of caregiving.

“You helped me understand why Dad was becoming more agitated, and gave me clear tools I could actually use. It made such a difference to both of us.”

Assistive Technology

Using simple tools to support memory, safety, and peace of mind. I help families choose and set up practical, dementia-friendly devices that ease everyday life.

“We didn’t know where to start with technology. You helped us choose what Mum actually needed, and now she’s more settled and we’re less worried.”

Understanding Care Needs

Compassionate assessments that clarify support needs and inform future planning. Especially helpful during transitions or when considering changes in care arrangements.

“Your report really helped us understand where Mum was struggling and how to move forward. It gave us language and options we didn’t have before.”

Mobility & Movement

Support for safe movement at home, if relevant. Advice on transfers, walking, or minor equipment recommendations on request.

“Even the small suggestions you made for safer movement have made a big difference to Mum's confidence.”
